This Pest Control Services Agreement ("Agreement") is entered into as of [Effective Date] between [Provider Name] ("Provider") and [Client Name] ("Client"), for the property at [Property Address].
Provider will treat for [Target Pests] using [Treatment Method], on a [one-time / Frequency] basis.
