This DJ / Entertainment Services Agreement ("Agreement") is entered into as of [Effective Date] between [DJ/Entertainer Name] ("Performer") and [Client Name] ("Client"), for an event on [Event Date] at [Venue], from [Start Time] to [End Time].
Performer will provide [Description of Services — e.g. DJ services, MC announcements], including [Number] hours of performance and [Setup/Breakdown Time].
Client will provide a music preference list, including any must-play and do-not-play songs, by [Date before Event]. Performer retains reasonable discretion to read the crowd.
Client agrees to pay [Amount], with a deposit of [Amount] due upon signing and the balance due [Timing — e.g. by the event date].
Performer will supply [Equipment List]. Venue must provide [Requirements — e.g. adequate power outlets, load-in access].
Performance time beyond the agreed hours is billed at [Overtime Rate] per additional hour or portion thereof, if agreed to on-site.
Cancellations more than [Number] days before the event forfeit the deposit only. Cancellations within [Number] days are billed at [Percentage]% of the total fee.
