This Employee Referral Bonus Agreement ("Agreement") is entered into as of [Effective Date] between [Company Name] ("Company") and [Employee Name] ("Employee").
Employee has referred [Candidate Name] for the position of [Job Title].
Employee is eligible for a referral bonus of [Amount] if the referred candidate is hired by Company and remains employed for at least [Number] days/months from their start date.
The bonus will be paid, less applicable withholdings, within [Number] days after the eligibility period in Section 3 is met, via Employee’s regular payroll.
No bonus is owed if the referred candidate was already known to Company, applied independently before the referral, or if Employee is involved in the hiring decision for that role.
