This Esthetician & Skincare Services Agreement ("Agreement") is entered into as of [Effective Date] between [Esthetician/Business Name] ("Provider") and [Client Name] ("Client").
Provider will provide [Description of Services — e.g. facials, peels, extractions] at [Location], for [Duration] per session.
Client confirms they have disclosed relevant skin conditions, allergies, medications (including retinoids/accutane use), and pregnancy status in [Attachment — Intake Form]. Provider may decline or modify treatment for Client’s safety.
[If applicable: For new products or treatments, Provider may require a patch test at least [Number] hours before the full treatment.]
Client agrees to pay [Rate] per session, due at the time of service, unless a package is purchased: [Package Terms, if applicable].
Appointments cancelled with less than [Number] hours’ notice, or missed without notice, are charged [Percentage]% of the session rate.
Client releases Provider from liability for reactions arising from undisclosed conditions or allergies. Provider’s total liability is limited to the amount paid for the session in question.
