This Event Equipment / AV Rental Agreement ("Agreement") is entered into as of [Effective Date] between [Rental Company Name] ("Provider") and [Client Name] ("Client"), for an event on [Event Date] at [Venue].
Provider will supply the equipment listed in [Attachment — e.g. speakers, microphones, lighting, projector].
