This Facility Use Waiver ("Waiver") is signed by [User Name] ("User") in connection with use of [Facility Name/Address] ("Facility"), operated by [Operator Name] ("Operator").
User agrees to follow all posted rules and staff instructions while using the Facility, including [Key Rules, if any].
User understands that use of the Facility (including its equipment and activities) carries inherent risks, including physical injury, and voluntarily assumes those risks.
User releases Operator, its employees, and agents from claims arising from use of the Facility, except for claims arising from Operator’s gross negligence or willful misconduct, to the extent permitted by [State / Jurisdiction] law.
Operator is not responsible for User’s personal belongings left at the Facility.
