This Group Fitness Class Agreement ("Agreement") is signed by [Participant Name] ("Participant") in connection with classes offered by [Studio/Instructor Name] ("Provider").
Participant is enrolled in [Class Type/Package — e.g. drop-in, monthly unlimited, X-class pack], valid for [Duration/Expiration].
Participant agrees to pay [Amount] for the package described above, or [Rate] per class for drop-ins.

Participant confirms they are physically able to participate in group fitness activities and voluntarily assumes the inherent risk of injury from participation.
Participant releases Provider from liability for injury arising from participation, except for injury caused by Provider’s gross negligence or willful misconduct, to the extent permitted by [State / Jurisdiction] law.
