This Hair & Beauty Salon Services Agreement ("Agreement") is entered into as of [Effective Date] between [Stylist/Salon Name] ("Provider") and [Client Name] ("Client").
Provider will provide [Description of Services — e.g. cut, color, styling, nails] at [Location].
Client confirms they have disclosed any allergies (including to hair dye/chemicals) or scalp/skin conditions relevant to the requested services.
Client agrees to pay [Rate], with a deposit of [Amount] required for chemical services (color, perms, extensions) to secure the booking, applied to the final price.
Appointments cancelled with less than [Number] hours’ notice, or missed without notice, forfeit the deposit (if any) and/or are charged [Percentage]% of the service price.
If Client is unsatisfied with a color or cut result, Client should notify Provider within [Number] days for a correction consultation, subject to [Correction Policy — e.g. free correction within X days, or a reduced fee].
Provider’s total liability for a service is limited to the amount paid for that service, except in cases of Provider’s gross negligence.
This Agreement is governed by the laws of the State of [State / Jurisdiction].
