This Massage & Spa Services Agreement ("Agreement") is entered into as of [Effective Date] between [Practitioner/Business Name] ("Practitioner") and [Client Name] ("Client").
Practitioner will provide [Description of Services — e.g. Swedish massage, facials] at [Location], for [Duration] per session.
Client confirms they have disclosed relevant health conditions, allergies, pregnancy status, or contraindications in [Attachment — Health Intake Form]. Practitioner may decline or modify services based on this information for Client’s safety.
