This Music & Art Lessons Agreement ("Agreement") is entered into as of [Effective Date] between [Instructor Name] ("Instructor") and [Client/Parent Name] ("Client"), for lessons for [Student Name] in [Instrument/Art Form].
Lessons are scheduled for [Duration] each, [Frequency], at [Location / Online].
Client agrees to pay [Rate] per lesson, or [Package Price] for a package of [Number] lessons, due [Timing].
Lessons cancelled by Client with less than [Number] hours’ notice are forfeited. Lessons cancelled by Instructor will be rescheduled or credited.
Client is responsible for providing [Instrument/Supplies], unless Instructor agrees to provide or rent equipment for an additional fee of [Amount].
