This Nanny / Babysitting Agreement ("Agreement") is entered into as of [Effective Date] between [Parent/Guardian Name] ("Family") and [Caregiver Name] ("Caregiver"), for the care of: [Children’s Names and Ages].
Caregiver will provide childcare on the following schedule: [Schedule], including [Duties — e.g. meals, homework help, light housekeeping related to the children].
Family agrees to pay Caregiver [Rate] per hour, paid [Frequency — e.g. weekly]. Overtime beyond [Number] hours/week is paid at [Overtime Rate].
In case of emergency, Caregiver will contact [Emergency Contact Name/Phone] and, if needed, call 911. Family authorizes Caregiver to consent to emergency medical treatment for the children if a parent/guardian cannot be reached, including: [Allergies/Medical Conditions/Medications to be aware of].
Caregiver agrees to follow the household rules described in [Attachment — e.g. screen time, meal guidelines, discipline approach]. Corporal punishment is not permitted.
