This Personal Chef Services Agreement ("Agreement") is entered into as of [Effective Date] between [Chef Name] ("Chef") and [Client Name] ("Client").
Chef will prepare meals according to [Menu/Meal Plan described in Attachment], on the following schedule: [Frequency], at [Client’s home / Chef’s kitchen].
