This Photo & Video Release Agreement ("Release") is signed by [Subject Name] ("Subject") in favor of [Company/Photographer Name] ("Company"), in connection with photos and/or video taken on [Date(s)] at [Location].
Subject grants Company the right to use their name, image, and likeness as captured in the photos/video (the "Materials") for [Purpose — e.g. marketing, website, social media, editorial use], in [Media/Territory/Duration — e.g. all media, worldwide, in perpetuity].
