This Sales Representative Agreement ("Agreement") is entered into as of [Effective Date] between [Company Name] ("Company") and [Representative Name] ("Representative").
Representative is authorized to sell [Products/Services] within [Territory — e.g. geographic area or accounts], on a [exclusive / non-exclusive] basis.
