This Yoga & Pilates Instruction Agreement ("Agreement") is entered into as of [Effective Date] between [Instructor Name] ("Instructor") and [Client Name] ("Client").
Instructor will provide [Private / Small-Group] sessions of [Duration], at [Location], on the following schedule: [Schedule].
Client agrees to pay [Rate] per session, or [Package Price] for a package of [Number] sessions, valid for [Expiration Period].
Client confirms they have disclosed relevant health conditions, injuries, or physical limitations in [Attachment — Health Questionnaire], and understands that physical practice carries inherent risk of injury, which Client voluntarily assumes.
Client releases Instructor from liability for injury arising from participation, except for injury caused by Instructor’s gross negligence or willful misconduct, to the extent permitted by [State / Jurisdiction] law.
Sessions cancelled with less than [Number] hours’ notice, or missed without notice, are charged as a completed session.
This Agreement is governed by the laws of the State of [State / Jurisdiction].
