Adventure & Outdoor Activity Assumption of Risk Agreement

1. Parties & Activity

This Assumption of Risk Agreement ("Waiver") is signed by [Participant Name] ("Participant") in connection with [Description of Activity — e.g. guided hike, kayak tour, zipline], organized by [Company Name] ("Company"), on [Date] at [Location].

2. Nature of the Activity

Participant understands this activity takes place in an outdoor/uncontrolled environment and involves risks including but not limited to [Activity-Specific Risks — e.g. falls, drowning, weather exposure, wildlife encounters, equipment failure], some of which may result in serious injury or death.

3. Physical Condition

Participant represents they are in adequate physical condition to safely participate, and will inform the guide of any relevant medical conditions or limitations before the activity begins.

4. Assumption of Risk

Participant voluntarily assumes all risks of the activity, known and unknown, and agrees to follow all safety instructions given by Company’s staff.

5. Release of Claims

Participant releases Company, its guides, and staff from claims arising from participation in the activity, except for claims arising from gross negligence or willful misconduct, to the extent permitted by [State / Jurisdiction] law.

6. Emergency Medical Authorization

Participant authorizes Company staff to arrange emergency medical treatment or evacuation if needed, at Participant’s expense unless otherwise agreed.
