Nutrition Coaching Agreement

1. Parties

This Nutrition Coaching Agreement ("Agreement") is entered into as of [Effective Date] between [Coach Name] ("Coach") and [Client Name] ("Client").

2. Services

Coach will provide general nutrition guidance and accountability coaching, including [Services — e.g. meal planning suggestions, habit tracking, check-ins], on the following schedule: [Schedule].

3. Not Medical or Dietitian Advice

Coach provides general wellness guidance, not medical nutrition therapy. 4. Health Disclosure
